Our private practice eye doctors offer the highest level of testing and will spend whatever time is needed to thoroughly evaluate a patient’s eye health and meet their needs.
Worth the Time, How Precious is Your Eyesight?
Eyes are extremely sensitive. Problems in other parts of the body can be detected early through the eyes. Diabetes, high blood pressure, tumors, liver and kidney problems and imbalances in the blood are all types of diseases that can be detected through the eyes. In fact, 16 different diseases can be detected, often before there are any symptoms. Private practice eye doctors have the skills and equipment to detect these diseases in addition to eye diseases. This level of care is now available through a vision plan from Vision Care Direct.

Benefit Summaries
Eye Health & Vision Care Exam:
Each member enrolled is entitled to a comprehensive eye health examination by a qualified Vision Care Direct Provider and includes refraction. This benefit does not include contact lens fitting fees. A $15 Member payment at Time of Service applies. For frequency of benefit see plans overview above.
Eyewear Materials – Lens and Frame Benefit:
A $15 Member payment at Time of Service applies. Lens Benefit: Each member is entitled to their choice of glass or plastic (CR-39) lenses in Single Vision, Bifocal, or Trifocal (FT25-28), or Lenticular. For Progressive Power lenses, member is entitled to apply the cost of Trifocal lenses to the cost of Progressive Power lenses. Member is responsible for the retail price difference. (Example: If Progressive lens price is $200 and Trifocal lens price is $120, member pays provider the difference of $80). Lens Enhancements such as high index, polycarbonate, tints, scratch resistant coatings, anti-reflective, etc., are available to member at customary charge. Polycarbonate lenses are recommended for children under the age of 18. For frequency of benefit see plans overview above.
Frame Benefit:
A $100 allowance towards any frame, member is responsible for amount exceeding $100. For frequency of benefit see plans overview above.
Sun Tint:
This is a solution dye tint on plastic (CR-39) lenses. Retail cost of solution dye tint may be applied to specialty glass and factory tints and dyes. Contact Lenses (Cosmetic): A $105 allowance towards contact lens materials. Contact lenses option is available in lieu of Lens and Frame option. No Member payment is required at the Time of Service. Diagnostic and fitting fees are not included in the Eye Health & Vision Care Exam. Contact lens professional, diagnostic and fitting fees are the member’s responsibility.
